The article describes the treatment of a group of patients who have been defined as being disruptive. This term is used to refer to patients that undermine and sabotage the therapeutic relationship and exhibit extreme levels of aggression directed at others, but even more frequently towards themselves. This (self) destructive behaviour is seen as an effort to excert control and increase the predictibility of the relationship with the therapist and others. The authors make the assumption that these clients are better understood from the perspective of a disorganised attachment style than through the appropiate DSM-IV categories. They examine therapeutic interventions for the attachment disturbances of these clients, for their claiming and idealisation, controling, disqualification and destructive behaviour.
